Summary of Problem.
a. History - The Oil Cooler Fan Shaft is an integral part of the helicopter tail rotor drive system. Recently
a report was received from the field indicating that during a 100 hour inspection it was noted the mounting
hardware for the oil cooler fan shaft (part number 70361-03009-047) was both improperly installed and im-
proper hardware was used. Six bolts are required and only three were installed, with none having a self-lock-
ing nut.

The purpose of this TB is to:
(1) Identify all H-60 aircraft that have not completed the 500 hour periodic inspection.
(2) Inspect aircraft with less than 500 hours for both improper, and un-installed oil cooler fan shaft
assembly forward mount hardware.

Inspection Procedures.
a. Inspect the aircraft DA Form 2408-13 to determine the aircraft total time.
b. If total time is greater than 500 hours and a PMS 2 has been completed, no further inspection is
required and the red horizontal dash / / - / / entry will be cleared.
c. If aircraft records indicate less than 500 hours and a PMS 2 has not been performed, inspect the
oil cooler fan shaft assembly forward mount hardware as follows:
(1) Gain access to the rear of the oil cooler fan duct..
(2) Remove oil cooler duct access cover.
(3) With a light source, inspect oil cooler fan shaft assembly forward mount flange attaching
hardware for the proper installation of 6 bolts, with the proper self-locking nuts IAW TM 1-1520-237-23-4,
paragraph 6-4-49.2.3..
