(1) Wholesale Stock -- Report receipt of message IAW paragraph 14c(1). Upon receipt of the
message and/or TB, Depot and Material Activity Commanders will ensure all serviceable items (Condition
Codes //A//, //B//, //C//, //D//, AND //E//) listed in paragraph 6 located in Wholesale Depot Storage shall be
placed in condition code //J// and tagged with a Suspended Tag/Label -- Material, DD Form 1576/DD Form
1576--1. Do not remove original condition tags. Report compliance with the message or this TB IAW para-
graph 14d(1).
(2) Retail Stock -- Report receipt of this message IAW paragraph 14c(2). Upon receipt of the
message and/or TB, Commanders and Facility Managers maintaining retail stock at installation level and be-
low shall contact the supported aviation unit to perform the procedures required IAW paragraphs 8 and 9 on
suspect material. Dispose of discrepant material IAW paragraph 10. Report compliance with this message

Summary of the Problem.
a. Lateral servo bellcrank assembly, P/N 70400--08150--045, manufactured by Purdy Machine Com-
pany (Cage Code 15152) completed preliminary strength testing in 1996. Results indicated that endurance
strength was significantly below that of the original equipment manufacturer. The component retirement life
was reduced to 360 hours, time since new, as outlined in Aviation Safety Action Message
UH--60--ASAM--97--06 (TB 1--1520--237--20--189 and TB 1--1520--250--20--14), to minimize impact to the
fleet and allow supply channels to accumulate adequate stockage levels. Scheduled retirement and equip-
ment attrition should have removed discrepant assemblies from use. However, several assemblies were re-
cently identified in use that had exceeded the allowable life limit of 360 hours.
